What You Will Need

  • Your Application ID
  • Your paper check or an email from OUR Florida confirming payment has been sent.

When should you fill out the form?

  • You have not received a check and it’s been 15 days or more since you received an email about a payment disbursement
  • If you received payment via check with an incorrect payee name
  • Your check was mailed to an incorrect address
  • Your landlord has changed since you applied for the program
  • You no longer need assistance and received a check

Checks that need to be corrected and reissued should be mailed to the Tallahassee hub at the address below.
